If you plan to utilize PTO 12/28/2025 – 1/10/2026, please complete this form no later than 1/10/2026.

HR/Payroll will enter this information into Paylocity on your behalf for the first payroll only.

Once PTO balances have been fully loaded into Paylocity, we will send a follow-up communication letting you know when you can begin independently entering PTO directly into Paylocity for the remainder of the year.
