If you notice a missed punch, incorrect time, or need to make an adjustment, Paylocity allows you to submit a time punch correction In the meantime, you don’t have to wait to access your benefits. You can easily view, download, or print a virtual medical ID card by following the steps below. Having the digital card handy is a great option for appointments, prescriptions, or anytime you need quick access.
How to access your virtual medical ID card:
- Visit the American Plan Administrator portal at apatpa.com
- Select Login, then choose Member
- Click Create Member Account
- Enter the following personal information:
- Legal First Name
- Legal Last Name
- Group Number: 4206
- Date of Birth
- Social Security Number
- Personal Email Address
- Create a unique password (minimum of 8 characters)
- Click Submit
- Once your account is confirmed, select Request ID Card from the dashboard
- View your ID card and print and/or save it as needed
We will send additional communication with details regarding dental and vision ID cards, as well as instructions on how to submit claims for STD, LTD, Critical Illness, Accident, and Hospital benefits once that information is available.
