If you plan to utilize PTO 12\/28\/2025 \u2013 1\/10\/2026, please complete this form<\/u><\/strong><\/a> no later than 1\/10\/2026<\/strong>.<\/p>\n\n\n\n

HR\/Payroll will enter this information into Paylocity on your behalf for the first payroll only.<\/p>\n\n\n\n

Once PTO balances have been fully loaded into Paylocity, we will send a follow-up communication letting you know when you can begin independently entering PTO directly into Paylocity for the remainder of the year.<\/p>\n\n\n\n
